Primary Care and Addictions Outreach

As part of the response to COVID-19 an outreach team has been working to help address care gaps for vulnerable and unsheltered people in Campbell River in the context of the current pandemic.

Funding for this work has been extended to the end of March 2021.

The team has been following a set schedule to provide outreach to various locations in Campbell River in collaboration with the MOUHSS (Mobile Outreach Unit for Health and Support Services). The MOUHSS (“Moose”) is a project created by the Kwakiutl District Council (KDC) and supported by the Division of Family Practice as well as other local sponsors. Services on board the mobile unit at present include social work, counselling, nursing (primary care nursing and connection to positive wellness), GP for urgent primary care (including COVID screening and swabbing) and addiction treatment.

Program Mandates:

1. Urgent primary care for vulnerable and unhoused people in Campbell River. The team aims re-establish any primary care connections that may already exist within the community.

2. COVID-19 screening and swabbing for vulnerable people.

3. Access to addiction care and treatment.

 

If family physicians have patients who they know to be experiencing homelessness or are otherwise vulnerable due to other social determinants of health, and who you feel may benefit from connection to the outreach team, you may call the outreach physician at 778-348-8933.

This phone will be answered from 8:30-12:00 on Mondays, Tuesdays, Thursdays and Fridays, and 12:30 – 4:00 on Wednesdays.

Alternatively, please feel free to share the outreach schedule with patients, who may “self-refer” by presenting to one of our locations (see schedule on p.2). All notes are copied to the MRP if the person has one. The notes are also available on PowerChart under “Pandemic/Epidemic consult”.
