Self-Management Health Coach Program

Self-Management Health Coach Program provides individualized telephone support for people living with chronic diseases such as diabetes, chronic pain, cancer, heart, kidney and lung diseases, arthritis, lupus, Parkinson, MS, etc.

Volunteer health coaches provide weekly 30 minutes telephone coaching for 3 – 6 months for individuals wanting to improve their confidence and motivation to become healthier and to better manage the daily challenges of living with chronic conditions. Individuals are paired with coaches based on gender and possibly shared chronic conditions.
