Community Health Services provides health care and personal support services to eligible people served by Island Health. Maximizing client wellness and their ability to care for themselves is our focus.
Our in-home and community based services support you to:
- Improve your health and quality of life, and
- Remain independent and in your own home for as long as possible
These settings may include hospital, but we can also offer you other options based on your needs, such as residential care and assisted living, doctors office or other primary care settings.
Our services add to, but do not replace, your efforts to care for yourself with help from family, friends and the community.
Services for eligible clients are delivered by teams of health care professionals in a variety of settings, and may include:

North Island Wound Care Collaborative
A team of health care professionals working in association with your family physician/nurse practitioner. We provide an integrated service of prevention, education, treatment and support related to chronic wounds. See attached brochure, under Resources for more information regarding this collaborative.
