Advance Care Planning

Advance Care Planning is a process of thinking about and sharing your wishes for future health care. It can help you tell others what would be important to you if you were ill and unable to communicate.

  • We will take you through a process of learning about different medical procedures and how they might or might not be of benefit to you.
  • We will discuss the need for a Substitute Decision Maker, a person to speak for you if you were unable.
  • We will encourage you to have this discussion about your wishes with your family, friends and physician.
  • Lastly, by writing these wishes down, and having the form witnessed, you will have completed your Advance Care Plan.

Most people and families are greatly relieved when this is completed. We provide lots of anecdotal examples and plenty of time for each person to ask questions as they pertain to them.

Classes are held at 440 Evergreen Road .   To Register or find out more information about the Advanced Care Planning sessions - please call the CR Hospice office at (250) 286-1121.
