“Chronic Disease Management/Out Patient Nutrition” Located in the Wellness Centre – at Campbell River Hospital. Clients can either be referred by their Physician or Self Refer to any of our services.
The Campbell River Hospital Wellness Center provides small group education sessions, targeted to chronic disease management patients, delivered by a variety of health care professionals including:
- Dietitians, Registered Nurses, Doctors, Pharmacists and Cardiac Technologists.
The CDM team (RN, Dietician, & Social Worker) are available to provide individual assessments, lifestyle coaching, education, goal setting, counseling and support.
